The Integrated Chronic Care model is based on the premise that chronic disease outcomes are better managed by addressing the needs of the whole person with a focus on self-management support.  With a team of interdisciplinary health care professionals, we integrate the physical, psychosocial and environmental needs of individuals by focusing on the person instead of the disease(s)/condition(s), improving functional health and quality of life.  The care team includes physicians, nurse practitioners, registered nurses, licensed practical nurses, physiotherapists, occupational therapists, clinical therapists, dietitians, and navigators.

This team is a seeking Physician to provide short term coverage during a period of active recruitment. Part time and full time options are available for qualified candidates. Options of 6 or 12 month appointments are negotiable. 

Job Description

The Integrated Chronic Care Services team is currently seeking a physician to provide short term coverage for this collaborative and interdisciplinary team of health professionals. Located in Halifax with a future move planned to Lower Sackville, this position will provide comprehensive assessments and care planning for individuals with complex chronic conditions such as:

Chronic fatigue syndrome (CFS)

Environmental illnesses including Multiple Chemical Sensitivity

Fibromyalgia

Post- COVID
This position reports to the Head, Department of Family Practice, Central Zone for their professional and clinical activities, and to the Director, Primary Health Care and Chronic Disease Management, Central Zone and the Manager, Integrated Chronic Care Service for administrative activities delivered for the Program.

Because of the nature of the patient population, this is a completely scent-free environment, and all team members must adhere to this in this practice.

Responsibilities

Responsibilities include but are not limited to:

examine patients and take their histories, order diagnostic tests and laboratory procedures; advise patients on health care, health promotion, and disease prevention

act as a patient’s advocate

consultation with other medical practitioners 

prescribe and administer medications and treatments

Compensation

This position is a 1.0FTE and is compensated through a sessional arrangement which is an hourly rate. As of April 1st 2025, the hourly rate is $174.00. Scheduling is flexible, but physicians are expected to have regular weekly office hours to support team-based care and support care continuity. 

Expected Hours

To be discussed. Part time and full time options are available. 6 - 12 month appointments are negotiable. Physician would be provided ample notice if the position were to be permanently filled. 

Qualifications

eligible for Licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS)

eligible for certification with the College of Family Physicians of Canada (CFPC)

Canadian Medical Protective Association (CMPA)

experience and certifications relevant to working with this patient population an asset
